What is body HIFU?
Body HIFU is a non-invasive, energy-based treatment that uses focused ultrasound to heat the deep layers of the skin at precise points, triggering new collagen and a gradual tightening of mild-to-moderate laxity. It is not liposuction, it is not a weight-loss method, and it does not remove loose skin.
The energy is focused below the surface, so the skin is not broken and there is no wound or crust. What changes is happening underneath: small points of controlled heat set off a repair response that builds collagen over the following months.
It works best as a refinement, not a rescue. On the right candidate it firms tone and tightens a lax area with no downtime; on the wrong one —real hanging skin, large fat volumes— it quietly underdelivers, which is why the honest work happens in the assessment, before anything is switched on.
Techniques Dr. Arístides Arellano uses
The value of HIFU depends less on the device and more on how each area is planned. These are the decisions that shape the treatment:
- Focal depth chosen per area. The ultrasound is focused at different depths depending on the goal: shallower for tone and texture, deeper to tighten the supporting plane. Energy is deposited at the focal point without damaging the skin surface.
- Mapping the area first. The zone is marked and the treatable tissue is counted before any pulse. A mildly lax arm is not treated like a post-pregnancy abdomen, and the number of energy lines is calculated, not improvised.
- Spaced sessions. New collagen takes months to form, so the plan is built around spaced sessions and a review at two to three months, when the result begins to show.
- An honest line against surgery. When there is genuinely excess skin —a hanging abdomen, arms with loose folds— the assessment says so and refers to the correct surgical option instead of selling sessions that will never reach it. See tummy tuck and liposuction.
How is it performed?
In a body HIFU session, conductive gel is applied, the area is marked, and the handpiece delivers pulses of focused ultrasound at the chosen depth. Each pulse creates microscopic points of heat deep in the skin; the surface is not broken, so there is no wound and no crust.
The sensation varies: most people describe brief heat and prickling that coincide with each pulse, stronger over bony areas. It usually needs no anaesthesia, and afterwards there may be mild redness and swelling that settle within hours to a few days. There is no downtime: normal routine resumes the same day.
The result is not immediate, and this is the most important thing to understand. The heat does not shrink the skin on the spot: what it does is trigger the formation of new collagen, a biological process that unfolds over two to six months. That is why HIFU is judged over the medium term and, depending on the area and degree of laxity, may be planned across more than one session.
Its limits must also be stated plainly: HIFU tightens mild-to-moderate laxity; it does not remove excess skin, it does not eliminate large volumes of fat, and it is not a weight-loss treatment. If the problem is truly loose skin or significant localised fat, the honest answer is surgical, not energy-based.
Every medical treatment —topicals, peels and energy-based devices included— carries risk and requires a prior assessment. In darker skin (Fitzpatrick IV–VI, the most common phototypes in Mexico) a poorly indicated procedure can darken the patch instead of lightening it. Results may vary according to each patient, their phototype and their individual triggers.
Who is a good candidate?
Body HIFU performs within a specific profile and disappoints outside it. An honest map:
- Probably yes: people near a stable weight, with mild-to-moderate laxity on the abdomen, arms, thighs, knees or buttocks, who want gradual tightening without surgery or downtime and hold realistic expectations.
- Also a good fit: someone who wants to refine a small area and firm the tone a little, or to maintain a result, without going to the operating room.
- Not for you if there is genuinely excess skin —a hanging abdomen after pregnancy or major weight loss, arms with skin that hangs—: energy does not fix that.
- Nor if you want to lose weight or remove abundant fat: body HIFU is not liposuction and not a slimming method. See liposuction.
- Postponed or avoided over metal implants in the area, with a pacemaker, active skin infection or lesion, and in pregnancy. All of this is reviewed at the assessment.
Recommended post-treatment care

Dr. Arístides Arellano is a plastic surgeon holding a specialty licence: he practises under a Mexican medical licence (D.G.P. 1125959) and a specialty licence in Plastic & Reconstructive Surgery (0002008), both issued by the Directorate General of Professions (DGP), which reissued his certificate of professional standing in 2025. His career began before Mexico’s present-day specialty-board system existed: he belongs to the founding generation of plastic surgery in Puebla, the second generation of a surgical tradition started in 1971 by Dr. Francisco Arellano Ocampo.
