Alma PrimeX: fat and laxity at once, without theatre — and how far it goes

The limit, before anything else

There are three sentences this device cannot deliver, and promising them is the most common way to cheat a patient without quite lying:

That said, within its range PrimeX is very good — and the range is wide: a great many people do not need theatre, and do need more than a cream.

• "It will get rid of your belly." No. Where there is genuine fat volume, the result liposuction gives is of a different order. PrimeX refines; it does not empty.
• "It will tighten your excess skin." No. Radiofrequency improves moderate laxity. A skin apron after pregnancy or major weight loss is cut: that is an abdominoplasty. No energy substitutes for a scalpel when skin genuinely is excess.
• "You will lose weight." No. It is contour, not scales. If body mass index is high, weight comes first; contour is worked afterwards, and worked better.

How it works: two mechanisms, one session

This is the logic of the device, and why it combines two technologies rather than one:

The combination matters because the two problems usually arrive together: almost nobody has localised fat with no laxity at all. Treating only the fat in skin that has already lost tone can leave the area laxer than before — the classic failure of single-mechanism platforms.

• UltraWave — transverse ultrasound. It acts on the adipocyte in the treated area. Released contents are progressively reabsorbed over the following weeks, by the normal metabolic route.
• UniPolar — radiofrequency. It heats the dermis in a controlled way and stimulates neocollagenesis. This is the tightening component, and it is slow: collagen remodelling builds across months, not sessions.
• Patented rotational massage, built into the handpiece, improving tolerance and evenness of treatment.

Areas, sessions and what it feels like

It treats the abdomen, flanks, thighs, buttocks, arms and chin. Sessions are short — 8 to 30 minutes depending on the area — and need no recovery: you leave and carry on with your day. It is felt as warmth and a deep massage. It does not hurt.

It is not a one-off session, and anyone selling it as one is selling smoke: it is worked in a series, and the number is defined at the consultation according to area and starting point.

The effect on fat begins to show over the following weeks. The tightening component takes months, because it depends on new collagen. And it holds with stable weight: the device reduces fat in an area; it does not prevent gaining new fat. Results may vary.

Where it sits against the alternatives

The useful question is not "is PrimeX good?" but "is it what I need?". The honest answer is usually one of these:

That this conversation is had with a plastic surgeon rather than a seller of sessions is the only real guarantee you have that a problem solvable by an operation will not be put into a ten-session course — or the reverse.

• Moderate localised fat + some laxity → PrimeX. This is its indication.
• Fat only, in a well-defined area, and you do not want a course → cryolipolysis.
• Missing muscle, not excess fat → StarFormer.
• Moderate fat + laxity, and you accept a fibre under the skin → body endolift, which is stronger and genuinely a procedure.
• Genuine fat volume → liposuction or HD liposculpture.
• Excess skin → abdominoplasty. There is no shortcut.

The Alma PrimeX (Alma, Israel) is used: body contouring with transverse UltraWave ultrasound against fat and UniPolar radiofrequency for collagen, with a patented rotational massage. Abdomen, flanks, thighs, buttocks, arms and chin, in sessions of 8 to 30 minutes.

Dr. Arístides Arellano is a plastic surgeon holding a specialty licence: he practises under a Mexican medical licence (D.G.P. 1125959) and a specialty licence in Plastic & Reconstructive Surgery (0002008), both issued by the Directorate General of Professions (DGP), which reissued his certificate of professional standing in 2025. His career began before Mexico’s present-day specialty-board system existed: he belongs to the founding generation of plastic surgery in Puebla, the second generation of a surgical tradition started in 1971 by Dr. Francisco Arellano Ocampo.
