What is Long Hair FUE?
The Long Hair FUE is an FUE (follicular unit extraction) hair transplant performed without shaving the donor area: the hair stays at its usual length before, during and after surgery. Follicles are extracted one by one with a micropunch, keeping their length, and implanted the same way into the recipient area.
The practical difference is social, not medical. Conventional FUE requires shaving the nape and sides to work, and that shaved zone takes weeks to disguise; during that time it is obvious the person had surgery. With Long Hair FUE the surrounding long hair immediately covers the extraction points, so the patient leaves the clinic looking normal and can return to work without the procedure being obvious. It is the option for those whose working or public life cannot afford a shave.
It is worth being honest about what does not change. Long Hair FUE is not a different kind of graft, nor does it deliver a different hair: it is the same FUE surgery, with the same transplanted follicle that keeps the genetic memory of the donor area and therefore grows for life. What it adds is discretion during recovery, in exchange for a slower, more meticulous execution —handling follicles with a long shaft without damaging them takes longer— and, usually, a somewhat lower number of grafts per session.

How is it performed?
It is outpatient surgery under local anaesthesia, in a session of 4 to 8 hours depending on graft count, after which the patient walks out the same day. The process has four stages. First, the hairline is designed: this is the artistic part and the one that most betrays bad work, so it is drawn irregular and natural, respecting face shape and age, knowing the patient will keep ageing. Second, the follicular units —with their long hair— are extracted from the unshaved donor area, follicle by follicle, with a micropunch that keeps the shaft intact. Third, the recipient sites are opened, defining angle, direction and density: a hair implanted at the wrong angle will grow the wrong way for the rest of its life. Fourth, each unit is implanted, single-hair units at the front for a soft, natural edge and two-, three- and four-hair units behind, where they deliver density. The long hair stays in view throughout the procedure.
As with any hair transplant, between weeks two and six shock loss occurs: the transplanted hair falls out and the follicle rests before growing again. This is normal and expected, and is explained in writing before surgery so that no one mistakes it for a failure.

Who is a good candidate?
- People whose social or working life cannot tolerate a visible shave: public figures, those who face clients, or anyone who does not want to announce that they had surgery.
