Blepharoplasty: at what age, and how long do the results last?

The question "is it my time yet?" is not answered by your birth date but by the mirror: excess skin weighing on the lashes, bags no cream erases, a tired look even after sleeping well. This guide explains when eyelid surgery is usually indicated and how durable its results are.

There is no right age: there is an indication

Blepharoplasty corrects two distinct problems that can coincide: excess upper-eyelid skin (dermatochalasis), which in advanced cases interferes with vision, and lower-eyelid fat bags, which create a permanently tired look.

The first is mostly an aging phenomenon, usually operated from ages 45–60 onward. The second has a strong genetic component: some 30-year-olds have marked inherited bags — and surgery can be perfectly indicated for them without "waiting for the age." The indication comes from anatomy, not the birth certificate.

The typical decades, case by case

  • 30s: almost always genetic lower-lid bags. For upper lids at this age, the consultation first rules out other causes (brow ptosis, allergies, thyroid).
  • 40s–50s: the most common range. Redundant upper skin that starts showing in photos and makeup, lower bags that deepen with fatigue.
  • 60s onward: functional cases — excess skin can reduce the upper visual field. Here surgery has a medical goal in addition to the aesthetic one.

How many years does the result last?

Blepharoplasty is among the most durable facial surgeries. The removed skin and fat do not "grow back"; what continues is the face’s normal aging.

In practical terms: upper-lid correction usually holds up well on the order of 10 to 15 years, and lower-lid correction often longer — many people never need bag surgery again. These ranges are orientative and results vary with skin quality, genetics and habits such as smoking and sun exposure.

What keeps aging (and what blepharoplasty does not fix)

It helps to know what this surgery does NOT do, because realistic expectations live there:

  • It does not stop brow descent — if the brow drops with the years, it can "load" the lid again even though the operated skin remains fine. In some cases the consultation points to treating the brow, not (only) the lid.
  • It does not erase crow’s feet or fine skin texture — complements like fractional CO₂ laser exist for that and can be combined with surgery when indicated.
  • It does not remove dark circles caused by pigment: blepharoplasty treats bags (volume), not skin color.

How it is decided at the consultation

Dr. Arístides Arellano examines brow position, skin quantity and quality, bag size and distribution, visual function when applicable, and general eye health. From that comes the plan: upper, lower or both; with or without laser complement; and the technique — including the transconjunctival route with no external scar for lower bags in firm skin.

The blepharoplasty reference price is on the price list; the exact quote for your combination is confirmed at the consultation.

Frequently asked questions

Is there a minimum age for eyelid surgery?

There is no fixed medical minimum for adults: anatomy provides the indication. Genetic bags can be operated from the 20s–30s if well established; age-related excess skin usually becomes an indication from 45 onward. Each case is confirmed at a consultation.

Can blepharoplasty be repeated years later?

Yes. If after 10–15 years aging produces new excess skin, a second surgery is possible and in fact more conservative, since less tissue is removed. Many patients never need it; it depends on skin, genetics and habits.

Does eyelid surgery remove dark circles?

It depends on what you call a dark circle. If it is a bag (protruding volume), that is blepharoplasty territory. If it is a sunken groove or dark skin color, classic surgery does not treat it — alternatives are assessed case by case.

Why do I still get wrinkles around my eyes after surgery?

Because blepharoplasty removes skin and fat but does not change skin quality or paralyze the muscle that creates crow’s feet when smiling. They are different problems with different solutions; fractional laser is one of the usual complements.

How do I know whether my problem is the lid or the brow?

Try this at the mirror: gently lift the brow with one finger to its youthful position. If the lid "excess" disappears, much of the problem is brow descent. The consultation measures this precisely and defines whether to treat lid, brow or both.

How long does the surgery take and under what anesthesia?

Usually 1 to 2 hours depending on whether one or both pairs of lids are treated, with local anesthesia plus sedation in most outpatient cases. The exact scheme is defined by your case and general health.
